Nursing homes must be open for 60 days and nursing homes for 90 days.
The DIA said it had applied for trusteeship only once before.

- Humboldt Wellness and Rehabilitation (aka Humboldt Care Center) in Humboldt
- Timber City Wellness and Rehabilitation in Makoketa (also known as Crestridge Care Center)
- Winterset’s Madison Wellness and Rehabilitation (also known as Winterset Care Center North)
The affected care homes are:
- Villa Cottage in Fort Dodge
- Winterset’s Madison Square Assisted Living and Memory Care
